What Happens in a Physiotherapy Session

Every first session takes 60 minutes. Every follow-up session takes 45 minutes. Here is the standard structure.

Minutes 1 to 10 - Assessment

History review, pain mapping, range of motion measurement, strength testing, posture analysis. The therapist confirms the diagnosis from the doctor's notes and adjusts the plan if anything has changed since the last visit.

Minutes 10 to 20 - Pain Modulation

Heat or cold therapy, ultrasound, transcutaneous electrical nerve stimulation (TENS), or interferential therapy. The aim is to reduce pain and muscle guarding so the active phase works better.

Minutes 20 to 35 - Manual Therapy

Joint mobilization, soft-tissue release, myofascial work, trigger-point release. The hands-on portion is what differentiates a good physiotherapy session from a gym workout.

Minutes 35 to 50 - Exercise Therapy

Targeted strengthening, stretching, proprioception and functional retraining. Exercises are progressed every 1 to 2 weeks based on response.

Minutes 50 to 60 - Home Programme

Three to five exercises to do daily at home. Demonstrated, practiced together, written down and emailed if required. The home programme is where 60 percent of recovery actually happens.

It depends on the condition. Acute back pain often resolves in 6 to 8 sessions. Frozen shoulder needs 12 to 18 sessions. Knee replacement rehab takes 20 to 30 sessions. Your therapist will give you a specific estimate at the first session and update it every 2 weeks based on progress.
Properly done physiotherapy should not cause significant pain. Some discomfort during stretching of stiff joints or during deep tissue work is normal and brief. If a treatment causes lasting increased pain, tell the therapist immediately so the plan can be adjusted.
Not strictly required, but a recent doctor's evaluation is helpful, especially if pain is severe or has lasted more than 4 weeks. If you walk in without a referral, the therapist will assess and may recommend an orthopedic consultation if your symptoms suggest a problem that needs medical evaluation.
In many cases, yes. Most early-stage back pain, neck pain, frozen shoulder, mild osteoarthritis and many sports injuries respond to physiotherapy alone. Surgery is needed when conservative treatment has been tried adequately and failed, or when the condition is structurally too advanced.
Most patients feel some improvement within the first 3 to 5 sessions. Significant pain reduction usually happens by week 2 to 3. Full recovery, including building back strength and confidence, typically takes 4 to 8 weeks for most conditions. Keep going even when you feel better - stopping too early is the most common reason for relapse.
